A copy of your Pilot Training Record (For PPL, it must indicate greater than 5 hours of flight training)*

A copy of your Valid medical certificate (Don't forget to sign it)
- If you have a medical renewal, please upload the original medical document (sticker or paper) and a copy of the renewal.
